When the integrity of the breasts is destroyed, it can be devastating for many women. Breast reconstruction is required for breast deformities caused by a congenital condition or an acquired deformity such as burn or cancer treatment. Dr. Azita Madjidi performs breast reconstruction surgery at her Houston, Texas practice, dedicating her time and skills to restore the breasts of women who are suffering from breast deformity.
Cancer treatment often requires partial or total removal of one or both breasts and leaves women with a major deformity. The feminine body image is disturbed and women often suffer a blow to self-esteem, confidence, and contentment. Available options are tissue expander/implant reconstruction, using your own tissues from the abdomen (e.g. TRAM, DIEP, SIEA) or back in the form of a flap, or a combination of both. Congenital breast deformities such as that found in Poland syndrome can be devastating for young women. From teenage years to adulthood, there are different options available to restore natural looking breasts. Dr. Madjidi will help you find the best option for you or your daughter's specific needs.
Each option for breast reconstruction will be described to you during your consultation with Dr. Madjidi. Depending on each patient's condition, health status, body habitus, previous or future therapies, and life style, one of the options in general fits you better.
Implant reconstruction can be started at the time of mastectomy or later by placing the implant or a tissue expander. Most of the time, a layer of acellular dermal matrix is needed for better support and protection. Tissue expander will be expanded weekly in the office until reached the desired fullness. About a couple of months later, it is replaced by the final implant as an outpatient procedure. This type of reconstruction is the fastest and has little down time. Implants are not lifetime devices and may need to be replaced at some point during your life. In selected cases, the implant can be placed at the time of mastectomy without passing by the expansion.
Once your own tissue is chosen to be used for reconstruction, a flap is taken from you abdomen and brought up to make a breast. Depending on the type of flap from the abdomen, surgery can take from five to 10 hours with five to seven days of hospital stay. If a flap from the abdomen is made once for the reconstruction of one breast, it cannot be re-used for the second side. In cases where the patient has had prior abdominal surgery, tummy tuck surgery, or liposuction where the blood supply to the flap has been disrupted, the abdomen cannot be used for breast reconstruction. Healing is longer when a flap is used from the abdomen. Besides the breasts, the abdomen will need to heal as well. Once the flap has healed and the swelling resolved, the shape will need to be adjusted to look like a breast and the opposite site may need to be lifted, reduced or augmented to match the reconstructed one.
